RULES TO ENSURE YOUR HEALTH & SAFETY

WHEN RIDING THE SEGWAYS

ALL PARTICIPATING GUESTS MUST:

• Follow the instructions of our trained staff at all times.
• Wear all the safety equipment provided at all times during the activity.
• Participate in the safety briefing; failure to do so will result in exclusion from the activity.
• Follow the route provided by Park Holidays staff.
• Agree to follow the instructions given by Park Holidays staff at all times.

All guests under the age of 14 must be accompanied by a parent or guardian over the age of 18 at all times.

WAIVER RULES

1. I am 18+ years old and wish to undertake the Park Holidays Segway activity (“the Activity”).
2. I understand that riding a Segway in a park environment has a significant potential for serious injury and that it is necessary to maintain control of the Segway at all times.
3. To reduce the risks, I agree to follow the Safety Rules that I have received with this document as well as all the instruction to be given to me by instructors before undertaking the Activity.
4. I am voluntarily participating in this Activity with the knowledge of the dangers involved and

I agree to accept any and all risks of loss, injury, or death.

1. I will wear all PPE that has been provided especially the High Vis Vest and Helmet which will be done up at all time.
2. If assuming responsibility for the supervision of any under 18year olds in this agreement, I declare that I am aged 18 years or older.
3. I agree that I am responsible for my own safety and that of any under 18 year olds that I have signed responsibility for below and I undertake to ensure that he/she/they read, understand and will follow the instructions set out in the Safety Rules and verbal instructions given.
4. I declare that if I am not the child(ren)'s parent or guardian I have the authority of the parent or guardian to sign this risk acknowledgement and disclaimer.
5. In the event of an accident or any damage, loss, injury or death I acknowledge that Park Holidays UK  will not be liable for any direct or indirect loss, damage or injury arising from or in connection with the Activities (except for death or personal injury caused by Park Holidays UK negligence) and I waive all and any claims against Park Holidays UK in this respect both for myself and for the children in my care.
6. I agree immediately to cease operation of the Segway if I am concerned in any way about its operation.
7. I certify that to the best of my knowledge neither I nor any children in my care have a medical condition which might make me or him/ her/them more likely to sustain an injury. To the best of my knowledge I and any children in my care are not pregnant.
8. I confirm that I am not under the adverse influence of drink or drugs.
